How the Pathway Works
- Request an appointment for your loved one.
- The patient completes appropriate consent and privacy authorization for family involvement.
- Green Health Clinic Ghana performs an appropriate clinical assessment.
- Medication reconciliation and available-record review are completed.
- Testing, referrals and treatment planning are arranged as clinically indicated.
- With patient authorization, designated family members can receive appropriate care updates and participate in planning.
Clear Boundaries Matter
An adult patient’s care remains subject to the patient’s consent and applicable privacy requirements. Green Health Clinic Ghana is not a substitute for emergency services, and urgent or emergency concerns should follow the appropriate local emergency pathway.
